The SPECIAL study is a single-center, randomized, controlled interventional feasibility trial in critically ill patients with acute kidney injury (AKI) requiring continuous renal replacement therapy (CRRT). The aim of this trial is to demonstrate the feasibility of protocolized filter-change strategies, including high protocol adherence and a clear separation in achieved filter lifespan between study groups.
In addition, the study examines the incidence of ICU-acquired infections in the early post-randomization period, alongside corresponding changes in systemic and urinary inflammatory mediator profiles. The results will inform the design of future multicenter confirmatory randomized controlled trials.
